Participant Agreement, Release of Liability, and Assumption of Risk

 

By completing this registration form, I hereby voluntarily allow my child[ren] participate in any activities or programs sponsored or organized by NuLife Christian Fellowship Church on or off its premises. I understand that participation in these event/activity involves inherent risks, which may include, but are not limited to, physical injury, illness, and property damage. In consideration of being allowed to participate, I hereby agree to the following:

 

• Release and Waiver: I, on behalf of myself, my child[ren] and my heirs, executors, administrators, and assigns, hereby release, waive, discharge, and covenant not to sue NuLife Christian Fellowship Church, its officers, employees, agents, and volunteers from any and all liability, claims, demands, actions, and causes of action whatsoever, arising out of or related to any loss, damage, or injury, including death, that may be sustained by me or my child, whether caused by the negligence of the releases or otherwise, while participating in any activities or programs sponsored or organized by NuLife Christian Fellowship Church on or off its premises.

• Indemnification: I agree to indemnify and hold harmless NuLife Christian Fellowship Church and its officers, employees, agents, and volunteers from any and all liabilities, claims, demands, actions, costs, and expenses, including attorney's fees, arising out of or related to my child's participation in any activities or programs sponsored or organized by NuLife Christian Fellowship Church on or off its premises.

• Photography and Publicity: I grant permission to NuLife Christian Fellowship Church to use photographs, videos, or other recordings of my child taken during the event/activity for promotional, educational, or other purposes without compensation.

• Severability: If any provision of this agreement is found to be unenforceable or invalid, the remaining provisions shall remain in full force and effect.

• Consent for Emergency Medical Treatment: I hereby grant consent to NuLife Christian Fellowship Church and its representatives to seek and provide medical treatment, including but not limited to medical assessment, diagnosis, treatment, and transportation, for my child[ren] in the event of a medical emergency during any activities or programs sponsored or organized by NuLife Christian Fellowship Church.

I HAVE CAREFULLY READ THIS AGREEMENT AND FULLY UNDERSTAND ITS CONTENTS. I AM AWARE THAT THIS IS A RELEASE OF LIABILITY AND A CONTRACT BETWEEN MYSELF (OR MY CHILD) AND NULIFE CHRISTIAN FELLOWSHIP CHURCH, AND I SIGN IT OF MY OWN FREE WILL. I ALSO AGREE THAT TYPING MY FULL NAME SERVES AS MY ELECTRONIC SIGNATURE
